I concur it is probably in the sample you supplied.
I muxed your .264 into a mkv and played it back with Media Player Classic - Home Cinema x64 1.5.2.3456 using CoreAVC I frame stepped through the footage and the imagery would mess up ever time the same way.
I've posted the source file on mediafire for you admin if you want to take a look yourself.

admin it's a 2 click procedure to get the file from sendspace. ("regular download" (big blue button) --> "click to start download..." (big button also)).
